Congratulations for having the desire to adopt a homeless pet. This is our application, not a contract to adopt. As noted on the Adoption Process page, this is the first step in the process. A successful adoption depends on finding the perfect pet for you & the perfect permanent home for the pet. So please be as complete & honest as possible. We apologize the application is long, but it is necessary to get as much information as possible to make the right match. When finished, submit the form online, mail, or fax.
NOTE! (1) WE RESERVE THE RIGHT TO VISIT THE HOME WHERE THE DOG WILL LIVE PRIOR TO ADOPTION
(2) WE RESERVE THE RIGHT TO APPROVE OR DENY ANY ADOPTION FOR ANY REASON AND THE RIGHT TO NOT DISCLOSE THE REASON
(3) YOU MUST BE AT LEAST 18 TO APPLY
(4) ALL HUMAN AND CANINE MEMBERS RESIDING IN THE HOME MUST MEET THE DOG PRIOR TO ADOPTION AND

Other Information:

Please understand since we don't know you, these answers are all we have to try to match you with a dog. The more explanatory, the better.
How much do you think it will cost you to care for this dog for one year? $ Can you afford it? Yes No
If some catastrophic thing happens, such as your dog accidentally gets out and gets hit by a car and the veterinarian tells you "He'll be fine, but it will cost thousands of dollars to fix," financially how will you handle it? PLEASE BE SPECIFIC. NOTE: We are NOT asking about the moral dilemma. We are ONLY asking specifically how you would handle it FINANCIALLY.
What plans do you have for training? We want to know what specific methods you will employ, not just who will train the dog & not what commands you will teach.
How will you discipline your pet if he does something you want to correct?
How will you handle the dog marking his territory inside the house?
How will you handle the dog eating a hole in furniture or chewing up something valuable?
How will you handle finding a potty accident you did not see the dog have?
There is NO perfect dog. Just like humans, dogs make mistakes. For example, a housebroken dog might go back to being not housebroken or the dog may get so attached to you he develops separation anxiety. If the dog you adopt makes mistakes after he comes to your house, what will be your course of action?
What will you do if after adoption, this dog does not get along with your current pets? If you have cats, what will you do if the dog tries to harm the cats? What if there are fights with your current dogs & this dog?
How many hours will this dog be alone (without humans) during the day? Will your dog be tied outside? Yes No
If yes, what type of tieing mechanism? What is the maximum length of time tied?
Where will the dog be when you are not at home? Where will the dog sleep?
Who will care for the dog when you vacation?
If you decide you no longer want the dog, what will you do with him/her?
Have you considered any health problems or risks to which you may be exposing your current pets? Yes No

The word "you" in the next 3 questions applies to all adults residing in the house.
Has anyone in your household EVER released or returned a pet to an animal shelter or rescue organization? Yes No
Has anyone in your household EVER given away a pet? Yes No
Has anyone in your household EVER sold a pet? Yes No
If yes to any of the last 3 questions, what were the circumstances?
How many months per year will you keep your dog on heartworm preventative?
